Clinical Data Scorecard

As part of our efforts to continuously improve patient outcomes, CHS introduced a clinical data scorecard to affiliated hospitals in July. The scorecard provides clear visibility into clinical outcomes and establishes a consistent measure of performance by tracking the hospital-based performance of 35 metrics across five domains. The domains include: Safety and Harm Reduction; Mortality and Infections; Patient Experience and Team Integrity; Service Line and General Operations; and Service Line Quality.

The data collected is the result of decisions made by clinicians and caregivers and is being used to identify high performing facilities and best practices being delivered across the organization.

Scorecard results are shared with hospital administrators and clinical stakeholders monthly to provide important insights into the quality, timeliness and appropriateness of care. It is also being used to create benchmarks that guide continued improvements toward best-in-class healthcare delivery.

Since its launch, the scorecard has already helped facilities improve their data hygiene and tracking, and over time, will identify opportunities for improvement and innovation in clinical areas like sepsis, surgical performance and infection.

Metrics will be evaluated once a year to determine if adjustments or updates are warranted based on CHS’ strategic focus at the time. The first evaluation is slated for late 2022, with any changes to be published early in 2023.

A list of the metrics tracked – organized by domain – can be found in this downloadable file. We hope you will take the time to review them. Insights from the scorecard are essential to building a health system that continually learns from and improves upon its delivery of quality care.
